Submit Your BMI-MINI License Application

To activate your license, please do the following:

  1. Print and fill out the application that is linked to below.
  2. At the top of the application, WRITE THE LIVE365 USERNAME AND ACCOUNT NUMBER UNDER WHICH YOU PURCHASED YOUR LICENSES.
  3. DISREGARD THE SUBMISSION/PAYMENT INFORMATION at the bottom of the application.
  4. Submit your application to Live365 via fax or via mail. Fax it to (650) 345-7497 ATTN: Licensing. Or, mail your application to:
